A = Excellent. The manager is pro-active in finding and resolving problems before I become aware of them

B = Good. Addresses and resolves problems in a timely manner

C = Satisfactory. Problems always get resolved but not as timely as I would like.

D = Doesn’t seem to be concerned with my problems. Will fix them according to his/her schedule.

F = Does not get problems resolved.

5. Do you have an unresolved concern?

A = No problem

B = No; I did and CCJ resolved it

C = Yes, but I have not yet informed CCJ about my concern

D = Yes, and I’ve informed CCJ but the concern is still unresolved
